ABOUT IMMUNIZATIONS

A REMINDER FOR 6TH GRADE STUDENTS:

THE STATE OF INDIANA REQUIRES THAT ALL INCOMING 6TH GRADERS BE COMPLIANT WITH THEIR IMMUNIZATIONS.   LETTERS WERE MAILED HOME TO THOSE STUDENTS WHO ARE NOT COMPLIANT.  PLEASE MAKE SURE THAT YOU HAVE THE SHOTS LISTED IN THE LETTER COMPLETED BY SEPTEMBER 22nd.  I WILL BE SENDING OUT EXCLUSION LETTERS AFTER THAT DATE FOR THOSE STUDENTS WHO DO NOT TURN IN PROOF THAT THE SHOTS WERE COMPLETED. THESE STUDENTS WILL BE EXCLUDED FROM SCHOOL ATTENDANCE UNTIL PROOF IS PROVIDED. REMEMBER THIS IS STATE LAW.

FOR 7TH AND 8TH GRADERS:

IF YOUR CHILD IS NOT COMPLIANT WITH SHOTS, YOU WILL BE RECEIVING A LETTER INFORMING YOU WHICH SHOTS YOUR CHILD NEEDS, AND THE DATE IN WHICH THEY NEED TO HAVE THE SHOTS COMPLETED BEFORE THEY ARE EXCLUDED FROM SCHOOL.

PLEASE REFER TO THE INDIANA STATE DEPARTMENT OF HEALTH FOR SPECIFICS ON IMMUNIZATIONS:
